Benefits information is not accessible to anyone other than the employee without written permission on file in Human Resources Management. There may be occasions, such as emergency surgery, when someone else needs access to the employee's benefits information. Once an Authorization for Release of Employee Benefits Information form is on file in Human Resources Management, benefits information can be released to the person(s) designated on the form.
