Just like a W-2, MSU is required to provide Form 1095-C to you for tax preparation purposes. Form 1095 is required as part of the Employer Shared Responsibility Provision in the Affordable Care Act. This form helps verify that you, your spouse, and any eligible dependent children were offered health insurance coverage from MSU. Some households will receive multiple forms if they were covered under different plans or changed plans during the year.
Important Notice Regarding Print Availability of Form 1095-C
On Dec. 23, 2024, the Paperwork Burden Reduction Act was signed into law which amended Section 6056 of the Internal Revenue Code. Effective immediately, MSU is no longer required to mail a print copy of Form 1095 to all employees, except upon request. Do I need Form 1095-C to file my taxes?
It depends. Your Form 1095-C may or may not need to be attached to your individual tax return, so you are encouraged to speak to your dedicated tax professional.
The Affordable Care Act previously imposed a tax penalty for those who didn't have health insurance or qualify for an exemption, however, this mandate was repealed by Congress in 2017 and took effect Jan 1, 2019 for all future tax years.
How do I obtain a copy of my 1095-C form?
Active employees will have access to your electronic Form 1095-C no later than March 3 in myState (Banner). View instructions to access your form in myState (Banner) now. You have the right to request a printed copy of your Form 1095-C by contacting human resources at benefits@hrm.msstate.edu or (662)325-3713 and it will be available for in-person pickup from human resources by the later of Jan. 31 or 30 days after the request is received.
Terminated employees may not have access to this portal and must request a copy of your form from human resources by contacting human resources at benefits@hrm.msstate.edu or (662)325-3713, if appropriate.

Giving Consent and Accessing Tax Documents Electronically
Federal regulations require that employees give their consent to receive their tax documents in an electronic format only. Once consent is given, it carries forward each year and does not need to be repeated.

Disclosure Notice
- An employee who consents to receive their 1095-C form electronically will not receive a paper copy.
- The Electronic 1095-C Consent Statement is effective for 1095-C forms issued for the current calendar year and all future periods until consent is withdrawn or the individual is no longer employed by Mississippi State University.
- After giving consent, an employee may still request a paper form by contacting the Department of Human Resources Management. The request for a paper form will not be considered as revoking consent to receive the form(s) electronically.
- An employee may withdraw their consent through the MyState portal in Banner and submitting the change. The withdrawal of consent does not apply to a form that was furnished electronically before the withdrawal of consent via MyState portal in Banner.
- Mississippi State University will not provide the form electronically to an individual who has terminated their employment prior to January 31. These individuals will receive paper copies via their department. Only active University employees will have access to the electronic version of the form. If a former employee requires an additional copy of the form, they may follow the aforementioned methods of contacting Human Resources.
- If an employee has consented to receive an electronic version of a form and has not printed the form prior to termination, they should contact Human Resources in order to receive a paper copy via U.S. mail.
- The form may be required to be printed and attached to a federal, state, or local income tax return.
