2024 Employee Benefit Guide
Benefits orientation is strongly encouraged and is held weekly in the HRM Training Room, 181 McArthur Hall. To enroll, contact HRM at (662)325-3713 or email us at benefits@hrm.msstate.edu You must reserve a spot to attend orientation.
Important note regarding electronic and digital signatures: All forms must be printed, completed, and returned with an original signature. We will not accept documents with electronic or digital signatures.
Submit your paperwork online - Online form submissions must include all necessary paperwork. New hire enrollment forms with incomplete paperwork will not be accepted or processed. Refer to the New Employee Benefits Forms Checklist for a list of all required paperwork.
Mandatory Forms
All new employees must complete and return the following mandatory forms within 31 days of their hire date.
- Authorization for release of employee benefits information
- Retirement Acknowledgement Form
- Pre-Tax Benefit Plan Enrollment Form
- State and School Employees' Health Insurance Plan Application for Coverage
- State Life Insurance provided by Minnesota Life Insurance
- Waiver of optional insurance coverage
- PERS or ORP retirement enrollment forms must be completed within 30 days of your hire date.
